Wednesday afternoon a crash on State Route 15 and Mansfield Road in Defiance County left 5 people injured. The Ohio State Highway Patrol says that they are continuing their investigation of the crash.

According to the authorities, Kari Reiman, 28, was westbound on Mansfield Road just before 3:20 pm when she failed to yield at the stop sign and crashed into 19-year-old Ashley Bemis who was heading southbound on State Route 15 with 41-year-old Tracy Palmer, 13-year-old Madison Palmer and Ashley's infant son. 

All 5 of those involved in the crash were transported from the scene by Life Flight to the hospital. The investigators have released that Madison was not properly restrained, having the shoulder strap of the seat belt behind her and only using the lap belt portion. She sustained injuries to her face and head.
